1. Collection of Personal Information
iReminder may collect the contact information (e.g., name, phone number and email addresses) and select demographic information (e.g., location, gender and age) in order to use our services. We also collect medication information to develop reminder and other messages (e.g., educational information, regimen instructions and motivational messages). We use this information to send you reminder and other messages and to contact you regarding updates or with other information regarding our services. It is your responsibility to maintain the accuracy of your contact and other personal information in your user account so that we may continue to send you reminders or other messages you have requested. You may notify iReminder via postal mail, email, facsimile, or telephone at the addresses or numbers set out above to request changes to your information. Such changes will be made within 24 hours of the request.
iReminder also may collect or track the home server domain names, email addresseses, type of client computer, files downloaded, search engine used, operating system, and type of Web browser of visitors to the Site, and aggregate and user-specific information regarding which pages Site visitors access.
When a visitor performs a search within the Site, iReminder may record information identifying the visitor or linking the visitor to the search performed. iReminder may also record limited information for every search request and use that information to solve technical problems with the services available directly or indirectly in connection with the Site and to calculate overall usage statistics.
2. Use of Personal Data Collected
Personal data collected by iReminder may be used by iReminder for many reasons, for example, for statistical analysis of users’ behavior, for product development, for content improvement, for fulfillment of a requested transaction or record keeping, or to customize the content and layout of the Site. Aggregate visitor data may be used for internal purposes, and individually identifying information, such as names, postal and email addresseses, phone numbers, and other personal information which visitors voluntarily provide to iReminder may be added to iReminder’s databases and used for future calls, mailings, and other communications regarding site updates, new products and services, and upcoming events.
3. How We Safeguard the Information you Provide to Us
Reminder has implemented various security measures to protect against the unintended loss, disclosure, misuse or alteration of the information under our control.
- You must log into the iReminder website any time you want to access your personal information. In order to view your account on iReminder's website, you will be asked to log in with a unique password. This password is personal to you and should not be shared with anyone. If you leave the iReminder website, you should be sure to log off. However, in the event that you forget to log off and leave your computer or if there is no activity on the iReminder website for more than 15 minutes, you will automatically be logged off and will be asked again to log in with your password.
- All of your personal information is encrypted when it is stored in and transmitted by our system. "Encrypted" means that it is scrambled with special complex codes so that no other computer can read it.
- You have full control over your information. You can change any piece of information in your password-protected My Profile section at any time or request that iReminder make changes via postal mail, email, facsimile, or telephone at the addresses or numbers above. Such changes will be made within 24 hours of the request.
- iReminder may use your information to contact you regarding account status and changes to the subscriber agreement, privacy statement, and any other relevant policies or agreements.
- Reminder monitors the limited number of our employees that can potentially access your information. As a part of general maintenance procedures that ensure the security of your personal information and the stability of our servers, there are a limited number of employees who may have access to our user database. These employees are required to abide by our Privacy Policy and are subject to strict disciplinary action including but not limited to termination of employment if this policy is breached. iReminder shall also alert the appropriate authorities if any actions warrant such notification.
- We will never ask for your password in an unsolicited phone call or email. Do not ever give it out.
- All the information iReminder collects is hosted on secure servers stored behind firewalls that are backed up regularly to ensure minimal loss of information in the event of a system failure or disaster. Unfortunately no data transmission over the Internet or any wireless network can be guaranteed to be 100% secure. As a result, while we strive to protect your personal information, we cannot ensure or warrant the security of any information you transmit to us or from our online products or services, and you do so at your own risk. Once we receive your transmission, it is in our interest to make best efforts to ensure its security on our systems.
NOTE THAT IREMINDER CANNOT PROTECT ANY INFORMATION ONCE IT HAS BEEN TRANSMITTED TO YOU. IF YOU SHARE A PHONE NUMBER OR EMAIL addresses WITH ANYONE ELSE (FOR EXAMPLE, A FAMILY MEMBER), INFORMATION TRANSMITTED TO THAT PHONE NUMBER OR EMAIL addresses MAY BE ABLE TO BE ACCESSED BY THAT OTHER PERSON. 4. Disclosure of Personal Data to Third Parties
Non-identifiable aggregate data may also be provided to customers and third parties for utilization review and other purposes.
Identifiable or non-identifiable data may be disclosed or distributed (1) to another party with which iReminder enters or reasonably may enter into a corporate transaction, such as, for example, a merger, consolidation, acquisition, or asset purchase, or (2) to a third party (a) pursuant to a subpoena, court order, or other form of legal process, (b) in response to a request by or on behalf of any local, state, federal, or other government agency, department, or body, whether or not pursuant to a subpoena, court order, or other form of legal process, or (c) if determined by iReminder in its sole judgment that such disclosure or distribution is appropriate to protect the life, health, or property of iReminder or any other person or entity.
iReminder is not responsible for the conduct, actions, omissions, or information handling or dissemination practices of clients who have contractural access to personal helath informaton and iReminder will not be liable therefor.
The Site may contain links to other World Wide websites or advertisements for or placed by third parties. iReminder is not responsible for the privacy practices of such websites, advertisers, or third parties, or for the content of such sites or advertisements. It is possible that these links or advertisements, themselves, may be used by third parties or others to collect personal or other information about Site visitors. It is solely the visitors’obligation to review and understand the privacy practices and policies of these other websites and of these advertisers and third parties.
5. Security Measures
iReminder has implemented numerous security features designed to prevent the unauthorized release of or access to personal information, which may include, without limitation, encryptions and firewalls. Please be advised, however, that the confidentiality of any communication or material transmitted to or from iReminder via this Site or email cannot be guaranteed. Accordingly, iReminder is not responsible for the security of information transmitted via the Internet. Instead of communicating with iReminder via email or the Internet, visitors can contact iReminder by mail, phone, facsimile, at the addresses or numbers set forth above.
6. Opt-Out Right
Except as necessary for iReminder to provide the services, information, or products requested by a visitor, the visitor may opt out of having his or her personally identifiable information, which has been voluntarily provided to iReminder through an online or paper form, registration form, survey, or other similar mechanism, prospectively retained by iReminder, used by iReminder for secondary purposes, or disclosed by iReminder to third parties by contacting iReminder via mail, email, facsimile or telephone, at the addresseses or numbers set out above. (This does not apply to collection of the type of Web browser of the visitor to the Site or to information provided or collected that does not meet the above description.)
7. Access to and Ability to Correct Personal Data

8. Right to Change Privacy Statement
iReminder may change this Privacy Statement at any time. Notice of any new or revised Privacy Statement, as well as the location of the new or revised statement, will be posted on the Site for at least 60 days after the change. It is the obligation of users visiting the Site before the change to learn of changes to the Privacy Statement since their last visit. Any change to this Privacy Statement shall be effective as to any visitor who has visited the Site before the change was made.
